Level 1 Support Coordination is focused on helping participants connect with the supports and services available to them.

This may include assistance with:

  • Understanding available supports.

  • Connecting with appropriate providers.

  • Building awareness of community and mainstream supports.

  • Developing confidence in accessing supports.

  • Establishing connections that support your goals.

  • Strengthening your ability to navigate your supports.

The focus: building connections and confidence.

SUPPORT COORDINATION — LEVEL 2

Coordination of Supports

Level 2 provides a more hands-on approach to coordinating your supports.

Depending on your circumstances, this may include:

  • Helping you understand and implement your supports.

  • Connecting and communicating with providers.

  • Coordinating multiple services and supports.

  • Identifying gaps or barriers.

  • Helping resolve issues that may arise.

  • Supporting communication between relevant parties.

  • Helping you work towards your goals.

  • Reviewing whether your current supports are working effectively for you.

  • Building your confidence and independence in managing your supports.

The focus: bringing the different pieces together.

SUPPORT COORDINATION — LEVEL 3

Specialist Support Coordination

Level 3 is designed for participants who experience more complex support needs or circumstances requiring a higher level of coordination.

This may involve:

  • Coordinating complex support arrangements.

  • Working with multiple providers and services.

  • Identifying and addressing barriers to supports.

  • Supporting communication across a broader support network.

  • Assisting with complex situations and changes in circumstances.

  • Developing strategies to improve the coordination of supports.

  • Helping build a more sustainable support network.

  • Working collaboratively with relevant professionals and services.

The focus: navigating complexity and creating a coordinated approach.
